What Is MIRENA® Used For?

MIRENA® is approved for use by premenopausal women and teenagers for two main reasons:
  • Preventing pregnancy – Effective for up to eight years, after which it must be removed.
  • Menorrhagia  – Effective for up to five years. If it’s still necessary, the IUD should be replaced.
It initially releases 20 mcg/day of Levonorgestrel, reducing the dosage. MIRENA® is NOT used for emergency contraception.

MIRENA® Contraindications

Patients with the following conditions should not use MIRENA®:
  • Known or suspected pregnancy
  • Uterine anomalies
  • Past or present progestin-sensitive cancers
  • Known or suspected malignancy of the uterus or the cervix
  • Active reproductive tract infections
  • Pelvic inflammatory disease, suspected or known, a history of PID
  • Postpartum endometritis
  • Uterine bleeding
  • Hypersensitivity to any MIRENA® components
Health practitioners must educate their patients about the risks of MIRENA®, including an increased chance of ectopic pregnancy (although it is lower than without contraception), pelvic inflammatory disease, severe infection, and sepsis, perforation, expulsion, and ovarian cysts.

MIRENA® Administration Technique

Before insertion, patients must be checked for endometrial pathology, pregnancy, and contraindications. MIRENA® should only be insured and removed by licensed and fully trained medical personnel by using an aseptic technique. Removal and insertion carry the following risks:
  • Infections and sepsis
  • Total or partial perforation of the uterine wall or cervix, embedment
  • Expulsion
The IUD has a 32 mm polyethylene T-body with a hormone elastomer core. It comes with a flexible 4.4 mm Bayer inserter through which it is delivered into the uterus using a single-handed technique. Once the IUD is released, it cannot be reloaded. The timing of the insertion depends on previous oral, intrauterine, dermal, vaginal, or injectible contraceptive methods, pregnancies, miscarriages, and abortions. During insertion and removal, patients may experience pain, dizziness, bleeding, and vasovagal reactions, including syncope, bradycardia, and seizures. Discuss pain management options with your patients. MIRENA® comes in sterile packaging that must nor be compromised during transport, delivery, or storage. MIRENA® Side Effects

The most common side effects of MIRENA® include: 
  • Changes in bleeding patterns, including scheduled and unscheduled bleeding

  • Breast pain
  • Back pain

  • Septic abortion
  • Miscarriage
  • Sepsis
  • Premature labor
  • Premature delivery
  • Virilization of the female fetus

MIRENA® vs. Competition

All intrauterine systems are over 99% effective as contraceptive methods. The MIRENA® IUD is the longest-lasting hormonal IUD, so it may be suitable for people who don't want to insert IUDs so frequently. Here is how it compares to the competition:
  • MIRENA® vs. KYLEENA – KYLEENA is effective for five years. It contains the same hormone, only in a smaller initial dosage of 17.5mcg daily. It’s 28 mm x 30 mm in size, smaller than MIRENA®. 
  • MIRENA® vs. SKYLA—SKYLA is effective for three years, contains a lower dosage of progestin, and releases 14 mcg per day. It’s also 28 mm x 30 mm in size.
  • MIRENA® vs. PARAGARD – PARAGARD is a copper IUD effective for up to ten years. It is not indicated for menorrhagia. At 32 x 36 mm, it is larger than MIRENA®. Smaller copper IUDs are available.
